About the exam
FDNY S-12 Citywide Sprinkler Systems Certificate of Fitness Exam
In New York City, sprinkler systems must be inspected, tested and maintained by a competent person who holds an FDNY S-12 Certificate of Fitness and is employed or contracted by the building owner. The S-12 (Supervision of Sprinkler Systems, Citywide) is issued by FDNY's Public Certification Unit after a computer exam at FDNY headquarters. The same study booklet also covers the S-15 foam-water certificate.
The exam is written from FDNY's free study booklet, revised March 2024, which draws on the NYC Fire Code Chapter 9, the Fire Department rules and NFPA 25 as adopted by the city. It covers the S-12 holder's duties and records, the tag and disc system for out-of-service systems, system types and components, valves and water supplies, waterflow alarms, hanging and bracing, fire pumps and the inspection, testing and maintenance frequencies.
This practice bank has 150 questions written from that booklet, and each one cites the section and page it comes from. The booklet tells candidates to check FDNY's website for a newer version before the exam, so confirm the revision date when you book.

Why people fail
The mistakes that cost people the FDNY S-12 Sprinkler Systems
- Not memorizing the inspection, testing and maintenance frequencies. Daily, weekly, monthly, quarterly, annual and five-year tasks are easy to mix up.
- Confusing the out-of-service tags and discs: red tag with white disc for a system fully out of service, red tag with blue disc for partial, and the yellow and green tags for deficiencies and good condition.
- Mixing up system types: wet pipe, dry pipe, pre-action, deluge and antifreeze systems, and the current antifreeze limits.
- Missing the numbers: spare head counts (6, 12 or 24 by system size), FDC clearances and the temperatures that drive cold-weather checks.
- Not knowing the notification duties when a system is impaired, including when FDNY must be told and when a fire watch is needed.
A study plan that works
- Download the March 2024 FDNY S-12 / S-15 booklet and read chapters 1 to 16 once.

- Take timed Tests (50 questions, 90 minutes). Once you score 85 percent, take a full-length run at your own pace to get used to 75 questions.
- Apply and pay online, and bring your ID and employer letter to FDNY headquarters.

Source: FDNY S-12/S-15 Study Material (rev. Mar 2024), Sec. 1, p. 12
Why: Glycerin may not exceed 48 percent and propylene glycol may not exceed 38 percent. The older booklet figures of 50 and 40 percent are outdated.

Source: FDNY S-12/S-15 Study Material (rev. Mar 2024), Sec. 12.5.1 D, p. 66
Why: Fully out of service is red tag and white disc. Partially out of service is red tag and blue disc, and a system free of defects gets a green tag.
Source: FDNY S-12/S-15 Study Material (rev. Mar 2024), Sec. 3, pp. 15-17
Why: The booklet requires at least 36 by 36 by 78 inches of working space around wall-mounted and free-standing FDCs. Smaller envelopes would obstruct a pumper hookup.
